Based on focus groups with service users and their families and friends an item pool was generated. The focus groups asked the question 'How do you know someone is better?' A total of 20 topics under 7 themes was generated.

Convergent validity‣ supported by stakeholder endorsement of all items as 'important 'or 'very important' by stakeholders who were service users, health care professionals and commissioners.

Describes the development of a 26 item scale measuring benefit from 12-step attendance. Construct validity‣ two factors accounting for 44.8-57.3% of variance interpreted as psychological wellbeing and absence of craving. Internal consistency‣ psychological wellbeing 0.91-0.93; absence of craving 0.83-0.92. Predictive validity‣ change greater with 12step involvement sampled at time points zero, 3, 6 & 12 months - the craving subscale accounts for 33.6% of effect on %days abstinent at 4-12months; psychological wellbeing has delayed effect.

Describes the development of a 50 item scale covering 10 domains of recovery. Construct validity‣ single factor loading 0.54-0.78 on the 10 domains. Internal consistency‣ test-retest reliability 0.50 (physical health) to 0.73 (abstinence). Convergent validity‣ ARC total score correlates with WHOQOL subscales WHOQOLphys 0.83, WHOQOLpsych 0.84, WHOQOLsoc 0.69, WHOQOLenv 0.82 with variation across domains of 0.43-0.87. Correlations with the Treatment Outcome Profile (TOP) ARCtotal with TOPtotal 0.40; ARCphys with TOPphys 0.35; ARCpsych with TOPpsych 0.39. Discriminant validity‣ ARC score cutoff ≥27.5 indicates recovery with sensitivity 92%, specificity 69%, and area under ROC 0.89.
